How to get your Google Form URL
- Open your Google Form in editing mode
- Click the Send button (top right corner)
- Click the link icon (chain link symbol) in the Send dialog
- Check "Shorten URL" to get a
forms.gle/...link — this creates a cleaner QR code - Copy the link and paste it into the generator above

Where to use a Google Form QR code

Set response settings before printing. In Google Forms → Settings, configure whether the form accepts one response per person (requires Google sign-in), allows editing after submission, and shows a confirmation message. Change these before the QR code is distributed — updating the form URL after printing means reprinting.
Check mobile layout. Scan your QR and complete the form on a phone before printing. Long text questions and file upload fields can be awkward on small screens. Keep forms short — 5 questions or fewer works best for on-the-spot scanning.
Response notifications. In Google Forms, turn on email notifications for new responses (Responses tab → three-dot menu → Get email notifications) so you're alerted the moment someone submits.
